PROBATE ADDENDUM: Affidavit of surviving spouse naming petitioner as designee

This affidavit is an attachment that can be submitted along with the probate form petitioning for transfer of a deceased person's personal property. It should be included with your petition if you are using the statement of the deceased person's spouse that names you as the person handling the transfer and distribution of that property.
